    Hammasperäinen poskiontelotulehdus määritellään paikalliseksi poskiontelon limakalvon paksuuntumiseksi, joka liittyy tulehtuneeseen tai laajasti hoidettuun hampaaseen tai suukirurgiseen toimenpiteeseen. Hammasperäinen syy saattaa olla taustalla 15 %:ssa poskiontelotulehduksista, erityisryhmien osalta jopa 40 %:ssa. Hampaan tukikudos- ja juurikanavatulehdukset aiheuttavat yli puolet hammasperäisistä poskiontelotulehduksista. Hammasperäiset tulehdukset ovat yleensä anaerobivoittoisia ja polymikrobisia. Tyypillisimmät oireet ovat toispuolinen märkäinen erite sekä tukkoisuus, paha maku tai haju, toispuolinen poskikipu tai -arkuus, liman valuminen nenänielusta, epämukava tunne kasvoissa tai ikenissä ja hammassärky. Kuvantamisessa käytetään panoraamakuvausta, intraoraalikuvia ja kartiokeilatietokonetomografiaa, joihin liittyy kuitenkin virhelähteitä. Hoito onnistuu, jos suun tulehdus hoidetaan. Hammashoidon lisäksi tarvitaan yleensä mikrobilääkehoito, usein poskiontelopunktio ja joskus sivuontelokirurgiaa.

    Matrix metalloproteinases (MMPs) such as gelatinases are differentially expressed in human tissues. These enzymes cleave specific substrates involved in cell signaling, tissue development and remodeling and tissue breakdown. Recent evidences show that gelatinases are crucial for normal dentin development and their activity is maintained throughout the entire tooth function in the oral cavity. Due to the lack of information about the exact location and activity of gelatinases in mature human dentin, the present study was designed to examine gelatinolytic levels In sound dentin. In situ zymography using confocal microscopy was performed on both mineralized and demineralized dentin samples. Sites presenting gelatinase activity were identified throughout the entire biological tissue pursuing different gelatinolytic levels for distinct areas: predentin and dentinal tubule regions presented higher gelatinolytic activity compared to intertubular dentin. Dentin regions with higher gelatinolytic activity immunohistochemically were partially correlated with MMP-2 expression. The maintenance of gelatinolytic activity in mature dentin may have biological implications related to biomineralization of predentin and tubular/peritubular dentinal regions, as well as regulation of defensive mechanisms of the dentin-pulp complex.

    Objectives In children and adolescents, cone-beam computed tomography (CBCT) is frequently used for localization of unerupted or impacted teeth in the anterior maxilla. CBCT causes a higher radiation dose than conventional intraoral and panoramic imaging. The objective was to analyze the location of impacted canines in a three-dimensional coordinate and thereby optimize the CBCT field-of-view (FOV), for radiation dose reduction. Materials and methods Location of 50 impacted maxillary canines of children under 17 years was retrospectively evaluated from CBCT scans. The minimum and maximum distances of any part of the right- and left-side canines to three anatomic reference planes were measured to assess the adequate size and position of a cylindrical image volume. Results A cylinder sized 39.0 (diameter)x33.2 (height) mm, with its top situated 13.8 mm above the hard palate, its medial edge 8.4 mm across the midline, and anterior edge 2.5 mm in front of the labial surface of maxillary central incisors fitted all the analyzed canines. Conclusions In this sample, the FOV required for imaging maxillary impacted canines was smaller than the smallest FOV offered by common CBCT devices. We encourage development of indication-specific CBCT imaging programs and aids to facilitate optimum patient positioning.

    Signaling centers, or organizers, regulate many aspects of embryonic morphogenesis. In the mammalian molar tooth, reiterative signaling in specialized centers called enamel knots (EKs) determines tooth patterning. Preceding the primary EK, transient epithelial thickening appears, the significance of which remains debated. Using tissue confocal fluorescence imaging with laser ablation experiments, we show that this transient thickening is an earlier signaling center, the molar initiation knot (IK), that is required for the progression of tooth development. IK cell dynamics demonstrate the hallmarks of a signaling center: cell cycle exit, condensation and eventual silencing through apoptosis. IK initiation and maturation are defined by the juxtaposition of cells with high Wnt activity to Shh-expressing non-proliferating cells, the combination of which drives the growth of the tooth bud, leading to the formation of the primary EK as an independent cell cluster. Overall, the whole development of the tooth, from initiation to patterning, is driven by the iterative use of signaling centers.
