    There is an increasing interest in exploring clinically relevant information that is present in body fluids, and extracellular vesicles (EVs) are intrinsic components of body fluids (?liquid biopsies?). In this report, we will focus on blood. Blood contains not only EVs but also cells, and non-EV particles including lipoproteins. Due to the high concentration of soluble proteins and lipoproteins, blood, plasma and serum have a high viscosity and density, which hampers the concentration, isolation and detection of EVs. Because most if not all studies on EVs are single-centre studies, their clinical relevance remains limited. Therefore, there is an urgent need to improve standardization and reproducibility of EV research. As a first step, the International Society on Extracellular Vesicles organized a biomarker workshop in Birmingham (UK) in November 2017, and during that workshop several working groups were created to focus on a particular body fluid. This report is the first output of the blood EV work group and is based on responses by work group members to a questionnaire in order to discover the contours of a roadmap. From the answers it is clear that most respondents are in favour of evidence-based research, education, quality control procedures, and physical models to improve our understanding and comparison of concentration, isolation and detection methods. Since blood is such a complex body fluid, we assume that the outcome of the survey may also be valuable for exploring body fluids other than blood.

    Background: Cytomegalovirus (CMV) management post-transplantation relies on quantification in blood, but inter-laboratory and inter-assay variability impairs commutability. An international multicenter study demonstrated that variability is mitigated by standardizing plasma volumes, automating DNA extraction and amplification, and calibration to the 1st-CMV-WHO-International-Standard as in the FDA-approved Roche-CAP/CTMCMV. However, Roche-CAP/CTM-CMV showed under-quantification and false-negative results in a quality assurance program (UK-NEQAS-2014). Objectives: To evaluate factors contributing to quantification variability of CMV viral load and to develop optimized CMV-UL54-QNAT. Study design: The UL54 target of the UK-NEQAS-2014 variant was sequenced and compared to 329 available CMV GenBank sequences. Four Basel-CMV-UL54-QNAT assays of 361 bp, 254 bp, 151 bp, and 95 bp amplicons were developed that only differed in reverse primer positions. The assays were validated using plasmid dilutions, UK-NEQAS-2014 sample, as well as 107 frozen and 69 prospectively collected plasma samples from transplant patients submitted for CMV QNAT, with and without DNase-digestion prior to nucleic acid extraction. Results: Eight of 43 mutations were identified as relevant in the UK-NEQAS-2014 target. All Basel-CMV-UL54 QNATs quantified the UK-NEQAS-2014 but revealed 10-fold increasing CMV loads as amplicon size decreased. The inverse correlation of amplicon size and viral loads was confirmed using 1st-WHO-International-Standard and patient samples. DNase pre-treatment reduced plasma CMV loads by > 90% indicating the presence of unprotected CMV genomic DNA. Conclusions: Sequence variability, amplicon length, and non-encapsidated genomes obstruct standardization and commutability of CMV loads needed to develop thresholds for clinical research and management. Besides regular sequence surveys, matrix and extraction standardization, we propose developing reference calibrators using 100 bp amplicons.

    High electric fields are present in a rapidly growing number of applications, which include elementary particle accelerators, vacuum interrupters, miniature x-ray sources, and satellites. Many of these applications are limited by the breakdown strength of the materials exposed to electric fields. Different methods have been developed to improve the quality of metal electrode surfaces, aiming to increase their breakdown strength. Not many systematical studies have been performed to provide a proper understanding of what contributes to the correlation between the breakdown strength and the quality of the surface. In this work, we apply a novel method for reducing vacuum arc breakdowns by cleaning the electrode surfaces with O and Ar plasma. The method can be used to alter the surfaces of the Cu electrodes in situ, i.e., without exposing them to air between the measurements. This plasma cleaning treatment is shown to reduce the number of surface impurities and to speed up the conditioning process of the samples under high-voltage pulses. Specifically, the first breakdown field was observed to increase by more than 90% after the plasma cleaning.

    Results are presented of large-scale Molecular Dynamics simulations of low-energy He bombardment of W nanorods, or so-called 'fuzz' structures. The goal of these simulations is to see if ballistic He penetration through W fuzz offers a more realistic scenario for how He moves through fuzz layers than He diffusion through fuzz nanorods. Instead of trying to grow a fuzz layer starting from a flat piece of bulk W, a new approach of creating a fully formed fuzz structure 0.43 mu m thick out of ellipsoidal pieces of W is employed. Lack of detailed experimental knowledge of the 3D structure of fuzz is dealt with by simulating He bombardment on five different structures of 15 vol% W and determining the variation in He penetration for each case. The results show that by far the most important factor determining He penetration is the amount of open channels through which He ions can travel unimpeded. For a more or less even W density distribution He penetration into fuzz falls off exponentially with distance and can thus be described by a 'half depth'. In a 15 vol% fuzz structure, the half depth can reach 0.18 mu m. In the far sparser fuzz structures that were recently reported, the half depth might be 1 mu m or more. This means that ballistic He penetration offers a more likely scenario than He diffusion through nanorods for how He moves through fuzz and may provide an adequate explanation for how He penetrates through the thickest fuzz layers reported so far. Furthermore, the exponential decrease in penetration with depth would follow a logarithmic dependence on fluence which is compatible with experiments. A comparison of these results and molecular dynamics calculations carried out in the recoil interaction approximation shows that results for W fuzz are qualitatively very different from conventional stopping power calculations on W with a similarly low but homogeneous density distribution.

    Steroid hormones are involved in many physiological functions such as stress response and the maintenance of salt-water balance and pregnancy. Concentrations of steroids in the body fluids are generally very low (below ng/ml). Steroid hormones are metabolically associated and changes in mutual concentration levels of different steroids may signify a disease. Methods that allow the measurement of various steroids simultaneously are of great importance in investigating the role of steroid metabolism for example in formation of cancer. The aim of this work was to develop a sensitive and selective method for simultaneous quantification of 16 steroids in plasma. Nano liguid chromatography-microchip electrospray ionization-tandem mass spectrometry (nanoLC-µESI-MS/MS) was used in order to achieve good sensitivity. C18 enrichment column and separation column, and an electrospray tip were integrated onto the chip that was used in this work. Mass spectrometric parameters were optimized by using a MS calibration and diagnostic chip. It was noticed that the structure of steroids plays an important role on how the compound behave in electrospray ionization. Steroids with 4,5-ene-3-one-structure had much lower limits of detection than steroids without conjugated double bonds (0,075-0,5 ng/ml and 5-25 ng/ml respectively). The chosen sample pretreatment method to extract the steroids from plasma did not work properly, because it was able to extract only a third of the compound's real concentration. Analysis of some compounds was also difficult because of the background noise coming from plasma. The method development was therefore decided to continue with eight steroids that were well detectable and had 4,5-ene-3-one-structure. The limits of detection were 0,075-0,5 ng/ml in biological matrix for these compounds. Eight knock out and seven wild type mouse plasma samples were analyzed using the validated method. The method was able to quantify aldosterone, corticosterone and androstenedione. Developed method did not meet all the aims of this work. Derivatizated compounds, different equipment or totally new method should be used in order to accomplish the aims.

    The 2014-2016 JET results are reviewed in the light of their significance for optimising the ITER research plan for the active and non-active operation. More than 60 h of plasma operation with ITER first wall materials successfully took place since its installation in 2011. New multi-machine scaling of the type I-ELM divertor energy flux density to ITER is supported by first principle modelling. ITER relevant disruption experiments and first principle modelling are reported with a set of three disruption mitigation valves mimicking the ITER setup. Insights of the L-H power threshold in Deuterium and Hydrogen are given, stressing the importance of the magnetic configurations and the recent measurements of fine-scale structures in the edge radial electric. Dimensionless scans of the core and pedestal confinement provide new information to elucidate the importance of the first wall material on the fusion performance. H-mode plasmas at ITER triangularity (H = 1 at beta(N) similar to 1.8 and n/n(GW) similar to 0.6) have been sustained at 2 MA during 5 s. The ITER neutronics codes have been validated on high performance experiments. Prospects for the coming D-T campaign and 14 MeV neutron calibration strategy are reviewed.

    Työssä tutkittiin rasvahappojen kerrostumista itämerennorpan (Phoca hispida botnica) traanissa ja tämän merkitystä rasvahappokoostumukseen perustuvassa ravintokohteiden arvioinnissa. Vertaamalla sisätraanin rasvahappokoostumusta plasman ja maksan rasvahappokoostumuksiin pyrittiin selvittämään siirtyvätkö jotkut tietyt ravinnon rasvahapot toisia tehokkaammin traanin sisäosiin, eli heijastelevatko jotkut sisätraanin rasvahapoista ravinnon rasvahappokoostumusta toisia paremmin. Itämerennorpan traanin eri kerrosten rasvahappokoostumusta verrattiin myös sen tärkeimmän ravintokalan, Itämeren silakan rasvahappokoostumukseen, jotta nähtäisiin minkä kerroksen koostumus muistuttaa eniten ravinnon rasvahappokoostumusta. Vertailun vuoksi työssä tutkittiin myös makeassa vedessä elävän saimaannorpan (Phoca hispida saimensis) traanin ja maksan rasvahappokoostumuksia. Lisäksi määritettiin plasma- ja maksanäytteiden kuljettaman tai lyhytaikaisesti varastoiman varastorasvan määrät. Tutkimuksessa käytettiin Perämerellä ammutuista itämerennorpista kerättyjä traani-, maksa- ja plasmanäytteitä, sekä kuolleina löydetyistä saimaannorpista kerättyjä traani- ja maksanäytteitä. Kudosnäytteiden rasvahappokoostumusten määritys tehtiin analysoimalla niistä valmistettuja rasvahappojen metyyliesteriseoksia kaasukromatografisesti (GC). Lipidiluokkakoostumukset puolestaan määritettiin korkean erotuskyvyn ohutlevykromatografialla (HPTLC). Analyysien tuloksia käsiteltiin tilastollisesti pääkomponenttianalyysin (PCA) ja sen tuloksia ohjatusti luokittelevan menetelmän (SIMCA) avulla, regressioanalyysillä, sekä laskemalla koostumusten euklidisia etäisyyksiä eri näytteiden välillä. Jokaisen yksilön traanille luotiin vertikaalinen rasvahappoprofiili toisiaan nahasta lihakseen seuraavien osanäytteiden rasvahappokoostumuksen perusteella. Itämerennorpan traanin kerrostuneisuutta tutkittiin nyt ensimmäistä kertaa ja sen havaittiin olevan rakenteeltaan kerrostunut, kuten on havaittu myös aiemmin tutkituilla kahdella norpan alalajilla. Aiemmista tutkimustuloksista poiketen keskitraani ei kuitenkaan eronnut rasvahappokoostumukseltaan merkitsevästi muista traanikerroksista. Sisä- ja ulkotraanin väliset rasvahappokoostumuksen erot olivat sen sijaan merkitseviä. Traanikerroksista sisätraani muistutti eniten itämerennorpan tärkeän saalislajin, silakan, rasvahappokoostumusta. Itämerennorpan kudosten rasvahappokoostumus erosi selvästi saimaannorpan kudosten rasvahappokoostumuksista. Näiden kahden alalajin ulkotraanit kuitenkin muistuttivat toisiaan rasvahappokoostumukseltaan sisätraaneja enemmän, mikä viittaa siihen, että niiden ulkotraanin koostumusta säätelevät samankaltaiset lämmönsäätelyyn liittyvät geneettisesti määräytyvät tekijät. Rasvahappokoostumusten alalajikohtaisista eroista huolimatta traanin rasvahappojen kerrostumistapa oli samanlainen molemmilla tutkituista alalajeista. Traanin vertikaaliset rasvahappoprofiilit olivat kuitenkin hyvin yksilöllisiä. Useiden rasvahappojen suhteelliset määrät plasmassa ja sisätraanissa korreloivat tilastollisesti merkitsevästi keskenään. Tämä tulos vahvistaa oletuksen, että viimeaikaisella ravinnolla on vaikutusta erityisesti sisätraanin rasvahappokoostumukseen ja, että sen perusteella voidaan saada tietoa eläimen ravinnosta. On kuitenkin huomattava, että tietyt sisätraanin rasvahapot ilmentävät ravinnon rasvahappokoostumusta toisia paremmin. Vaikka traanien vertikaaliset rasvahappoprofiilit antavat mitä ilmeisimmin yksilökohtaista tietoa eläinten ravinnosta ja aineenvaihdunnasta, niitä ei ole pystytty tulkitsemaan aiemmissa tutkimuksissa kovinkaan syvällisesti. Nämä tulokset voivatkin osaltaan auttaa tulkitsemaan traanin vertikaalisia rasvahappoprofiileja tulevaisuudessa. Saatuja tuloksia voidaan lisäksi hyödyntää myös hylkeiden ravintokohteiden arvioinnissa käytettävien näytteenottoprotokollien suunnitteluun ja kehittämiseen.
