   <dim:field mdschema="dc" element="description" qualifier="abstract" lang="fi">Tämän tutkimuksen taustalla on henkivakuutusyhtiöiden tarve arvioida asiakkaidensa elinaikoja, sillä kerättyjen vakuutusmaksujen ja maksettujen etuuksien on oltava tasapainossa. Maksettujen etuuksien määrä riippuu voimakkaasti asiakkaiden eliniästä ja kuolinsyistä. 

Tutkimuksessa esitetään ja todistetaan kilpailevien kuolinsyiden teoreema. Todistus esitetään yksityiskohtaisesti. Keskeinen havainto on, että kilpaileviin kuolinsyihin liittyvä tutkimus painottui 1990-luvulle, mutta vuodesta 2000 alkaen tutkimuksen painopiste on siirtynyt kuolinsyihin yleisemmin. Kuolinsyiden kirjaamiskäytännöt kuolintodistuksissa ja eri kuolinsyiden välisten yhteyksien huomioiminen ovat keskeisiä syitä siihen, miksi kilpailevien kuolinsyiden lähestymistapaa käytetään nykyään vähemmän. Siitä huolimatta kuolinsyyt ovat edelleen hyödyllinen mittari aktuaareille sekä sosiaali- ja terveyspoliittisessa päätöksenteossa. 

Työssä on tarkasteltu kolmen eri maan kuolinsyitä syvällisemmin. Aineisto on käytettävissä vain melko karkealla aggregaattitasolla, mutta tästä huolimatta havaitaan maiden välisiä eroja. Länsimaissa kuolleisuus on laskenut lähes kaikissa ikäluokissa tasaisesti viimeisen 50 vuoden aikana. Itä-Euroopan maissa samanlaista kehitystä ei ole tapahtunut. Kuolleisuuden vähenemisen pääasiallisena syynä länsimaissa on lääketieteen kehitys ja elintapojen parantuminen, jotka ovat vähentäneet sydän- ja verisuonisairauksia. 

Työssä on valittu tarkempaan tarkasteluun maita ja tapaustutkimuksia, joiden kautta tuodaan esiin merkittäviä kehityssuuntia, kuten mielenterveys- ja hermostosairauksien yleistyminen, alkoholin, tupakan ja huumeiden käyttö sekä sosiaaliturvajärjestelmien merkitys.
Kilpailevien kuolinsyiden käsite toimii hyvin, jos kuolinsyiden välinen riippumattomuusoletus pitää paikkansa. Koska tämä oletus harvoin toteutuu, kuolinsyihin perustuva tarkastelu on usein houkuttelevampi vaihtoehto. Se on yksinkertaisempi ja tuottaa samansuuntaisia tuloksia.</dim:field>
   <dim:field mdschema="dc" element="description" qualifier="abstract" lang="en">The motivation for this study is based on the needs of life insurance companies to estimate the life times of their customers because the premiums collected and the benefits paid to the insurance holders have to be in balance. The benefits paid heavily depend on the length of life and the causes of death of customers. 

We introduce and prove the theorem of competing causes of death. The proof is carried out in detail. Our key observation is that the research is in the 1990s in competing causes of death, but after the 2000 the emphasis of the research has been in causes of death. The recording of causes of death in death certificates and associations between causes of deaths are the main
reasons that the competing causes of deaths approach has remained less used. Still, the causes of death are good metrics for actuaries and social and health and care politicians. 

We have selected causes of death of three countries under a more careful study. We are only able to look the cases at a very rough level of aggregation. Nonetheless, we can see differences between countries. In western industrialized countries, the mortality rate has decreased almost evenly during the 50-year period in all age cohorts. In eastern European countries, mortality rates have not decreased in the same period in the same way. The main reason for the decrease in mortality rates in western countries is that the development of medicine and improved living habits of the people have reduced heart and circular diseases. In closer study, we find many variations of patterns in diseases and causes of death. We have selected some countries and case studies to show these differences and important developments, as increase of mental and nervous diseases, use of alcohol, tobacco and drugs, and the importance of the state of social systems.  

The concept of competing causes of death only works well, if the assumption of independence between causes of death holds. Since this assumption rarely holds, the causes of death approach becomes appealing as it is less complicated and gives similar information in results.</dim:field>
